Fill out the Department of Labor forms. These can be printed out and mailed or the application can be made on line.
If you need help filling out the forms or you have questions, your local Resource Center can assist you.
EECAP can also assist you by answering questions, filling out forms or, in some instances, acting as your authorized representative.
